Rehabilitation under Resolution No. 309 with State Funding at MCRN
Resolution No. 309 is a state program that funds the rehabilitation of children with disabilities.
Under this program, children who have a disability certificate receive government funding once a year for rehabilitation in the amount of ₴16,000 or ₴25,000.
How to apply?
1. Contact us for a free consultation with our coordinator and to schedule a preliminary rehabilitation date at MCRN.
2. Prepare the required documents:
-Passport of one parent or guardian;
-Taxpayer Identification Number (RNOKPP, formerly ITN);
-Child’s birth certificate;
-Child’s passport (if aged 14 or older) and their RNOKPP;
-Disability certificate;
-Medical extract Form 027 (from the outpatient medical record);
-Individual Rehabilitation Program (IRP);
3. Visit your local Social Protection Office (OSZN) or the Administrative Services Center (CNAP) to submit an application for rehabilitation funding under Resolution No. 309.

4. After funding is approved, your documents will be sent to us by OSZN or CNAP. Your coordinator will call you to finalize details and schedule a neurologist consultation and EEG (if needed) to determine an individual treatment plan.
Important: Rehabilitation funding is allocated based on the order of application submission. We recommend applying for Resolution No. 309 funding at the beginning of the year (January) to ensure timely approval.

How to Obtain a Disability Certificate for a Child?
STEP I – Document Collection & Submission:
✔Visit a pediatrician who provides primary medical care or the healthcare facility where your child is observed.
✔Undergo a medical examination (either inpatient or outpatient).
As a result, you need to obtain one of the following documents:
✔Specialist’s advisory opinion (Form 028/0);
✔Medical record extract from inpatient treatment (Form 027/0).
Important! These documents must include:
-Medical justification for recognizing the child as having a disability;
-The diagnosis and corresponding medical code.
– Submit these documents to the pediatrician or relevant specialist to receive a referral for the Medical Advisory Commission (LCC).
-The doctor prepares and submits the documents for review by LCC to determine if the child qualifies for the category “Child with Disability” or “Child with Disability, Subgroup A.”
STEP II – Medical Advisory Commission (LCC) Evaluation:
✔LCC examines the child and reviews the documents within 7 working days from the date of submission. After review, LCC issues an official conclusion confirming whether the diagnosis qualifies for state social assistance.The official date of disability cognition is the date when the documents were received by the commission.
STEP III – Social Assistance & Benefits Registration:
✔Apply to the Social Protection Office (UPSZN) to receive state social assistance. Submit the Individual Rehabilitation Program (IRP) and the medical certificate confirming disability along with identity documents. Provide a photo of the child for the disability certificate issuance.
Important! Social assistance will be paid from the date of the parent’s application submission. It is advisable to submit your application as soon as possible, even if some documents are missing.
